Abstract: Adverse events associated with the drug Interactions are some of the common reasons behind the hospitalization across the world but on the other hand these can be prevented easily with an effective patient centered system Diverse mechanism have been successfully limited the occurrences of DDIs There are no cost effective mechanism which have resulted in effective manner within the setting This study aims to monitor the frequency of potential DDIs in tertiary care settings and to check the association among the diverse factors like poly-pharmacy and demographics
