Abstract: newline Abstract: newlineBackground: Katigraha, characterized by Vata and Kapha imbalance or Ama, manifests as Katishula and Katistambha, primarily affecting the joints of Sphika and Pristavanshasthi. This condition shares similarities with chronic lower back pain and Lumbar Spondylosis, a prevalent issue linked to sedentary lifestyles. Since 2005, lower back pain has been a leading cause of disability. Svedana, particularly Ashmaghna sveda, is a traditional treatment, albeit challenged by resource constraints. A modified approach, termed Modified Ashmaghna Sveda, was explored herein. newlineObjectives: This study aims to evaluate the efficacy of Modified Ashmaghna Sveda and Choorna Pinda Sveda in managing Katigraha and to compare their effectiveness. newlineMethods: Sixty-eight Katigraha patients were randomly assigned to Choorna Pinda Sveda or Modified Ashmaghna Sveda groups. Treatments were administered daily for 30 minutes. newlineResults: The Modified Ashmaghna Sveda group shows significant results with an 18.036% improvement in ODI, 41.428% improvement in VAS, and 24.813% improvement in 2MWDT. As compared to the Choornapinda sveda group which shows a 14.920% improvement in ODI, 30.686% improvement in VAS, and 10.153% improvement in 2MWDT. Therefore, it can be stated that Modified Ashmaghna Sveda demonstrated superior clinical outcomes when compared to Choorna Pinda Sveda, showing significant improvements in ODI, VAS, and 2MWDT scores. newlineInterpretation and Conclusions: Modified Ashmaghna Sveda exhibits comparable efficacy to Choorna Pinda Sveda in managing Katigraha. While statistically non-inferior, clinically, it outperforms Choorna Pinda Sveda.
