The Efficacy of Murivenna Ointment in the Management of Parikartika Fissure in Ano a Randomized Controlled Trial

Abstract

Introduction: Fissure-in-ano is a disease which commonly occurs in ano-rectal region. By considering ano-rectal diseases it is the one that affects more peoples (about 30 to 40%). Fissure in ano creates more inconvenience to the patients by affecting the routine life of the patients. People irrespective of their gender and age suffer with fissure in ano. The term fissure newlinecommonly denotes a split or a crack. It is characterized by a superficial tear in the lower part of the anal canal . 1 Fissure in ano is compared with Parikartika in Ayurvedic literatures. In this, patient experiences pain similar to that of cutting like pain with scissors in anal region. The Vrana(wound) which is formed in Parikartika is due to self causative factors in origin. Anal sphincter tonicity in high manner is considered as the main patho-physiology in case of fissure in ano. This causes the formation of fresh cut linear ulcer in anal canal. The most probable location of ulcer in anal canal are in the anterior or posterior part. . 2 Treatment aspect is commonly focused towards the anal sphincter muscle tonicity reduction and thus to increase the newlineproper blood supply to particular region for promoting proper healing of ulcer. Both modern science and Ayurvedic science are having plethora of treatment protocols. Modern management includes internal sphincter s medical manipulat ion and to correct the constipation. Snehavasthi(Oil enema therapy), Avagaha Swedana(medicated luke warm water sitz bath) and Lepanam ointments) are some of the commonly used Ayurvedic managements. Due to the symptoms like cutting and burning type of pain its clears the involvement of Vata and Pitta newlineDosha in Parikartika. Hence the drugs selection which has actions as Vata or Pitta Dosha alleviation and wound healing properties are effective in fissure in ano management. Murivenna is a medicated oil which is widely used in the conditions like contusions, fresh wounds, ano-rectal diseases and even for fractures .
