Abstract: INTRODUCTION newlinePeriorbital melanosis is a commonly encountered yet unexplored aesthetic condition in the Indian population. It is considered a phenomenon within the limits of physiology, thereby posing as a major concern for women with significant impairment of their quality of life. Various therapy methods may be used to effectively reduce pigmentation with periorbital melanosis, with superficially chemical treatments being the most preferred option, particularly those containing AHAs. Nevertheless, there is a lack of research in scientific journals that compare different therapy methods for periorbital melanosis in terms of their safety and effectiveness up to this point. Combination of lactic acid and trichloroacetic acid has shown encouraging results for periorbital melanosis but however,has not been compared to the efficacy of glycolic acid peels. newlineAIM newlineThe objective of this study is to evaluate the clinical effectiveness, security, and tolerance of 20% glycolic acid peels and 15% lactic acid with 3.75% trichloroacetic acid peels in treating congenital periorbital melanosis in Indian clients. newlineMATERIALS AND METHOD newlineThe research comprised an overall of 66 patients who met the selection parameters. A comprehensive record was obtained. Group A patients had a treatment of 5 sessions of 20% glycolic acid, with a 3-week break between each session. Patients in Group B had a treatment regimen consisting of five sessions of 15% lactic acid and 3.75% trichloroacetic acid peel. Each session was spaced three weeks apart.Both groups were instructed to use sunscreen that protects against a wide range of ultraviolet (UV) rays and to refrain from being directly exposed to sunlight. The evaluation of pigment reduction was assessed using clinical means, and patient grading was also conducted. newlineRESULTS newlineBoth the groups showed good improvement based on clinical grading, physician and patient assessment. However, on comparing both the groups, 15%lactic acid with 3.75% trichloroacetic acid was more efficacious ,with most patients showing improvement of 50% to 75% . The 20% glycolic acid group showed better global tolerance when compared to the other group.Most of the patients in both groups did not experience any side effects,Though statistically insignificant,more side effects in the lactic acid and TCA group were recorded. newlineCONCLUSION newlineBoth 20% glycolic acid peel and 15%lactic acid with 3.75%TCA peel are effective for the treatment of periorbital melanosis. However, in comparison, the lactic acid and TCA group showed better efficacy. 20%glycolic acid peel was shown to be of better tolerability. The incidence of side effects is slightly more in the lactic and TCA group. newline
