Abstract: INTRODUCTION: Acoustic force impulse elastography is a relatively new method that is now being utilized to differentiate between cirrhotic liver and non-cirrhotic portal fibrosis, which is present in a significant number of patients. Acoustic force impulse elastography is a technique that can be utilized to evaluate the stages and grades of fibrosis, and its effectiveness is evaluated in comparison to the results of liver biopsy. newlineAIMS AND OBJECTIVES: In order to analyze non-cirrhotic portal fibrosis/banti syndrome, the primary objective of the study is to differentiate it from cirrhosis with portal hypertension by utilizing acoustic radiation force impulse elastography. Additionally, the study will compare the results of the assessment with previous liver biopsy findings. The purpose of this study is to evaluate the diagnostic accuracy of shear wave ARFI elastography conducted on the liver and spleen in order to identify non-cirrhotic portal fibrosis, differentiate it from cirrhosis with portal hypertension, and compare the results with those obtained from liver biopsy. newlineMATERIALS AND METHODS: The radiology department was the location of a cross-sectional study that was carried out over the course of nine months. Patients was examined using B mode ultrasonography and will be subjected to elastography to monitor the stiffness of the liver (LS) and the spleen (SS), as well as the ratio of LS to SS will be calculated by using shear wave elastography velocity. Sampe size was thirty patients. Curvilinear probe in elastography setting was used for the study newlineRESULTSandCONCLUSION: We infer, With the use of Acoustic Radiation Force Impulse elastography (ARFI-E), diagnostic accuracy can be achieved. variables among cases with Liver cirrhosis has comparably better predictive value followed by of ARFI-E variables among cases with Non-Cirrhotic portal fibrosis, ARFI-E cases with Esophageal varices and ARFI-E variables when attempting to forecast Portal hypertension among patients with HVPG. newline
