Abstract: | The study was a prospective observational study, conducted in the department newlineof obstetrics and gynecology of, Chettinad hospital and research institute. In 100 newlinewomen diagnosed as unhealthy cervix were consecutively sampled in to the study. All newlinethe participants underwent, pap smear, colposcopy examination and biospsy of newlinesuspicious lesions. The ability of Pap smear and colposcopy in identifying the newlinemalignant disease/dysplasia was assessed by calculating sensitivity, specificity and newlinepredictive values of each of the methods. IBM SPSS version 21 was used for newlinestatistical analysis. newlineRESULTS: newlineHPE appearance showed CIN I, CIN II and CIN III in 8%, 5% and 4% of the newlineparticipants respectively. When compared gold standard (HPE), PAP smear had a newlinesensitivity of 88.2% (95% CI was 65% to 96%), specificity was 74.7% (95% CI was newline64% to 82%), False positive rate was 25.3% (95% CI was 17% to 35%) and False newlinenegative rate was 11.8% (95% CI was 3% to 34%). The Positive predictive value and newlineNegative predictive value were 41.7% (95%CI 27% to 57%) and 96.9.2% (95% CI newline89% to 99%).The Diagnostic accuracy was 77% (95%CI 67% to 84%). When newlinecompared to gold standard (HPE), Colposcopy had a sensitivity of 64.7% (95% CI newlinewas 41% to 825, specificity was 97.6% (95% CI was 91% to 99%), False positive rate newlinewas 2.4% (95% CI was 0% to 8%) and False negative rate was 35.3% (95% CI was newline17% to 58%). The Positive predictive value and Negative predictive value were 84.6% newline(95%CI 57% to 95%) and 93.1% (95% CI 85% to 96%).The Diagnostic accuracy was newline92% (95%CI 85% to 95%). newlineConclusions: newline1. PAP smear had higher sensitivity and lower specificity, when compared to newlinecolposcopy. That means the colposcopy had lower false positive rate and can newlineresult in lower unnecessary treatment rates, when compared to Pap smear. newline2. The positive predictive value of colposcopy was considerably higher with newlinecolposcopy, when compared to Pap smear. The negative predictive values of newlineboth the modalities were almost comparable with minimal difference. newline3. The reliability of colposcopy was considerably higher when compared to Pap newlinesmear in the study population newline |
