Abstract: Aim: To compare the efficacy of chemiluminescence with lugol s iodine newlineversus with toluidine blue in diagnosing dysplastic changes in tobacco newlineassociated oral lesions. newlineObjectives: To test the diagnostic efficacy of chemiluminescence with lugol s newlineiodine in diagnosing dysplasia in tobacco associated oral lesions. To test the newlinediagnostic efficacy of chemiluminescence with toluidine blue in diagnosing newlinedysplasia in tobacco associated oral lesions. To evaluate which combination newlineof the above is more sensitive and specific in detecting dysplastic changes. newlineMethodology: 84 patients with clinically suspicious oral lesions and oral newlinepotentially malignant disorders with tobacco habit history were done and they newlinewere randomly assigned to either of the diagnostic study group (Group A: newlineChemiluminescence with Lugol s iodine or Group B: Chemiluminescence with newlineToluidine Blue). All these patients were subjected to histopathological newlineexamination (Reference standard). newlineResults: The sensitivity, specificity, positive predictive value, negative newlinepredictive value and ROC were 91.7 %, 66.7 %, 84.6 %,80 %,0.792 for newlinechemiluminescence with lugol s iodine group (p=0.001) whereas for newlinechemiluminescence with toluidine blue , the sensitivity, specificity,positive newlinepredictive value(PPV) and negative predictive value(NPV) were 100 %, 60 newline%,93.3 %, 100 %, 0.800 (p=0.002) respectively. The combination of newlinechemiluminescence with toluidine blue had a better diagnostic efficiency in newline8 of 161 newlinedetecting dysplasia in tobacco associated oral lesions when compared to the newlinegroup of chemiluminescence with lugol s iodine. newlineConclusion: newlineIn literature there have been various studies where they have used these vital newlinestaining, optical diagnostic tools as efficient chair side adjuncts. When newlinechemiluminescence is used in combination with vital staining, the sensitivity, newlinespecificity, PPV, NPV has increased and this can be a valuable diagnostic newlinemethodology for screening of dysplasia in a wide spectrum of tobacco newlineassociated oral lesions. newline newline
