Abstract: Most of the diabetic patients in India, fail to achieve treatment targets and have higher newlinemean of HbA1c indicating a poor glycemic control, which may lead to micro and macro vascular complications. A better understanding of diabetes mellitus, and its complications may suggest effective planning for improving the Economic, Clinical, and Humanistic Outcomes (ECHO). Comprehensive diabetes care is a complex task that takes the whole team of healthcare professionals including the pharmacist to work together to provide multidisciplinary care for patients. We have aimed the evolving role of clinical pharmacist in the management of diabetes drug therapy, diabetes care and diabetic education. It helps to optimize the drug therapy for achieving the better therapeutic outcomes. This study would definitely create the awareness about the management of diabetes mellitus. The result of this study showed that the implementation of the pharmaceutical care practice (PCP) or pharmacist physician collaborative practice (PPCP) could result in good glycemic control compared to usual care practice (UCP). Clinical pharmacist coordinates with the physician during drug therapy and applies the clinical expertise in identifying, resolving and preventing drug related problems by providing information which is of valuable aid in accurate, safe and cost effective use of drugs. This study results gives a broad idea about pharmacist role on diabetes care and education by screening, monitoring diabetic treatment goals, assessing patient s health status and giving counseling for medication adherence which will help in optimizing the blood sugar level and enhancing the quality of life. We have aimed and developed a protocol to manage the Type II Diabetes Mellitus and its related complications. Since the tool developed with small and single ethnic population it needs wide spread data to become an adoptive model once this model is successfully developed this will be an easy tool to handle Type II Diabetes Mellitus. newline newline
