Abstract: | Spondyloarthritides (SpA) are a group of inflammatory arthritis which newlineconsist of Ankylosing spondylitis (AS), reactive arthritis, arthritis/spondylitis newlineassociated with psoriasis (PsA), and arthritis/spondylitis associated with newlineinflammatory bowel diseases. Assessment of Spondyloarthritides International newlineSociety (ASAS) has defined MRI methods for the assessment of sacroilliac joints and newlinespine, criteria for inflammatory back pain and developed new criteria for newlineclassification of axial and peripheral Spondyloarthritides. There is a long delay, newlineapproximately 5 - 6 years, between the first occurrence of the SpA symptoms and the newlinediagnosis of the disease especially for women, juvenile onset or HLA-B27 negative newlinepatients. HLA B-27 positivity is extremely relevant to the early diagnosis of SpA. newlineFive to 10% of the population are HLA B-27 positive and in patients with AS and newlineSpA the positivity of HLA B-27 changes to 70% to 95% and nearly 70%, newlinerespectively. Possible links between an HLA-B27 induced unfolded protein response newlineand activation of the Interleukin-23/Interleukin-17 axis have been observed in animal newlinemodels and may contribute to the development of the Spondyloarthritides phenotype. newlineSeveral genetic, experimental and clinical studies have evidenced that SpA was newlinetriggered by pathological activation of the IL-23/IL-17 axis newline |
