Abstract: | Background:Part 1 aims to explore possible factors which influence QoL of COPD and Part 2 evaluates the efficacy of circuit training program over supervised walking program in improving functional capacity and quality of life scores in COPD. Methods:Part 1 was a cross-sectional analytical study. Demographic factors,lifestyle and disease severity parameters; physical, physiological and mental factors recorded.SGRQ-C,SF 12, CAT scores were used to assess QoL.Correlation coefficient, bivariate analysis and multiple linear regression analysis were carried out. Backwards stepwise multivariate regression was used to determine independent effects of significant bivariate predictors. Differences in indices of HRQoL with different levels of COPD severity (according to GOLD) were tested using one-factor ANOVA. In Part 2 of the study was an observer blinded; parallel group, Randomized Controlled Trial with allocation ratio of 1:1. 52 patients were equally divided into two groups of 26 each. Pre-intervention 6MWD, PSQI and SGRQ-C scores were recorded.Results: Part 1: On bivariate analysis,6MWD,mMRC, depression, anxiety, and sleep disturbances were significantly correlating with the quality of life scores. (p valuelt0.01) Linear regression analysis showed that 6MWD and sleep scores were the only two factors significantly correlated with quality of life scores.Part2:QoL scores in CT group statistically improved from 49.51 to 39.48 (p valuelt0.01) and in SW group from 50.83 to 44.05 (p valuelt0.01). On comparing mean difference of QoL score of 10.03 compared to 6.78 which was significantly better in circuit training (p value=0.01), so also was the mean 6MWD improvement. (p value=0.01) newlineConclusion: Functional capacity as measured by 6MWD and SLEEP (PSQI) were the two strongly correlated factors with QoL in patients with COPD and Simple, minimal equipment oriented Circuit training of 8 weeks duration led to better improvement in QoL, sleep scores and 6MWD in patients with COPD. |
