Abstract: | INTRODUCTION: - COPD is a leading cause of morbidity and mortality worldwide it can have a considerable impact on the lifestyle of patients and their families, causing reduced mobility,social isolation, depression and anxiety and results in an economic and social burden that is both substantial and increasing. Commonly managed by multi-dimensional approaches as pulmonary rehabilitation including conventional physiotherapy, yoga and standard treatment of pharmacotherapy.AIM and OBJECTIVES:-The general aims of the thesis were to know the individual as well as comparative effect of home based physiotherapy versus yogic kriya in COPD.METHODS:- Randomized-controlled comparative study were included 150 COPD patients based on inclusion and exclusion criteria and divided into three groups (Group A; Physiotherapy Group (PG), Group B; Yogic kriya Group (YG) and Group C; Control Group (CG) in each group, n= 50). Physiotherapy Group and Yogic kriya Group were received exercises(using Elastic band) and yogic kriya (using Ujjayi pranayam) with medication and control group received medication (medical management) only. Outcomes were measured at baseline and 1st,2nd, 3rd, 6th, 12th month follow up for Primary outcomes i.e. Spirometry values (FVC% predicted, FEV1% predicted, FEV1/FVC ratio) and Secondary outcomes i.e. SGRQ, 6MWT,DASS21, chest expansion measurement, reduction in rate of acute exacerbation, emergency visit and hospitalization. RESULTS:- In our study out of 150 patients total 76 patients (PG n=27, YG n=25, CG n=24)completed the study including follow-up. At the end of 12 month, result shown significant improvement in Group A; P0.05, as compare to Group B and Group C for lung function, QOL, exercise capacity (6MWT), mood and psychological distress (DASS21), chest expansion,reduction in rate of acute exacerbation, emergency visit and hospitalization in patient with COPD.CONCLUSION: -Our study concludes that long term home based physiotherapy and yogic kriya both are effective in improving lung function |
