Abstract: Objective The objective of the study was to designed to understand the HIV newlineresistance pattern in (i) New HIV reactive patients (ART naive patients) (ii) newlineHIV reactive patients on ART for and#8805;1 year newlineMethod All the patients who were HIV positive as per NACO guidelines newlinewere recruited in the study after proper counseling and consent and were newlinegrouped as per objective of the thesis. Separate aliquots were made for newlineestimation of CD4 count, PVL and HIV genotype resistance testing. PVL was newlinedone by CavidiExaVir an economical, reliable and cheap alternative newlinemethod for detection of viral load in recourse limited settings. HIV genotype newlineresistance testing was done by a well-designed reliable economical in house newlinemethod targeting the specific pol region of the HIV.The samples were newlineoutsourced for sequencing and the final results obtained were then analyzed newlineusing Stanford HIV DR database. newlineResults Only 34 samples were amplified and successfully genotyped. Thirty newlineone out of 34 samples belonged to group of patients who were HIV drug newlinenaïve while 3 patients were, on ART. All patients samples were subjected to newlinegenotype testing for detection of drug resistance mutation in the Reverse newlinetranscriptase region of HIV 1. The drug resistance mutation patterns against newlineNRTI and NNRTI among study population were recorded. newlineConclusions This study emphasizes on the resistance pattern developed newlineby HIV on first line antiretroviral therapy. Although HIV transmission has been newlinecontrolled across the globe it is still a major threat to the control of HIV newlinetransmission. Hence an attempt was made to know the occurrence of drug newlineresistance in the patients attending a tertiary care hospital in Mysore. newlineCarrying out this kind of studies at regular interval in developing countries will newlinehelp in understanding the drug resistance pattern in HIV from time to time at newlinevarious geographical areas. These studies will help in changing the newlineantiretroviral regimen if deemed necessary newline
