Abstract: Leptin an adipocytokine shows its pleiotropic role which is beyond appetite suppression and management of energy expenditure exact role of leptin over nutrition inflammation vascular calcification in CKD patients on hemodialysis and not on hemodialysis was not clearly understood to study the association of serum leptin levels with malnutrition inflammation and abdominal aortic calcification in pre dialysis chronic kidney disease and maintenance hemodialysis patient A total of 207 patients among which 99 were pre dialysis CKD and 108 hemodialysis patients were included Institutional ethics committee approval obtained for the study demographic details and other data such as height weight BMI MAC TSF and MAMC creatinine albumin phosphorus Leptin calcium BUN HsCRP Total protein TIBC iPTH cholesterol triglycerides LDL and HDL were analysed Nutritional status assessed using malnutrition inflammation score abdominal aortic calcification using Lateral lumbar x ray and presence of arterial stiffness using pulse wave velocity Among 207 patients 156 patients completed the reassessment for MIS AAC PWV analysis leptin HsCRP at one year 43 patients expired six patients were initiated on HD 1 patient underwent renal transplantation and one patient lost the follow up before completing the reassessment Serum Leptin is significantly elevated in pre dialysis CKD than in hemodialysis patients in our study Leptin is significantly associated with arterial stiffness Serum leptin is an independent predictor of mortality in both groups Higher leptin levels are associated with better survival Leptin exhibits bimodal effect in pre dialysis and hemodialysis patients newline
