Abstract: India one of the worlds most populous nations also has a high incidence of impaired glucose tolerance IGT the condition that predisposes to Type II diabetes mellitus A number of diabetes control programs have been in operation throughout the country with a few targeting the prediabetic group with IGT Acanthosis nigricans AN the velvety hyperpigmentation of the neck has been identified as a marker for Insulin Resistance IR which underlies the impaired glucose tolerance This thesis was conducted to evaluate the utility of AN grades in detecting the presence and severity of insulin resistanceA prospective followup of a cohort of IR subjects with AN was conducted to study improvement in AN grades secondary to lifestyle modification and whether there was a corresponding improvement in impaired glucose tolerance at the end of six months Metformin an oral hypoglycemic agent was administered to patients with AN secondary to IR in addition to Lifestyle modification but no additional benefit was ascertained The oral glucose tolerance test was compared with serum insulin levels to check the accuracy of the former in detecting IR High improvement rates were noted in AN grades in subjects who regularly followed the lifestyle modification and lost weight irrespective of medication intake But subjects who defaulted on lifestyle modification saw poor outcome ratesAdiponectin is an adipocyte specific cytokine secreted by the adipose tissue with insulin sensitizing antiatherosclerotic and antiinflammatory properties Acanthosis nigricans AN is velvety hyperpigmentation of the skin flexures common in obesity and insulin resistance IR Studies suggest that serum adiponectin is lowered in IR also and could play a therapeutic role init
